Haemorrhoids
These are the enlargement and prolapse of the vascular cushions in the anal canal. The most common symptom is painless, bright red bleeding. They are graded from one to four according to prolapse, and treatment follows this grading:
- Grades 1-2: A high-fibre diet, warm sitz baths, medication that strengthens the vein wall, and office procedures such as rubber band ligation
- Grades 2-3: Laser haemorrhoidoplasty, Doppler-guided haemorrhoidal artery ligation
- Grade 4: Conventional haemorrhoidectomy
Anal Fissure
A superficial tear in the anal canal. Severe pain during and for hours after bowel movements, with a small amount of bright red bleeding, is typical. In the early stage it may heal with stool softeners, warm sitz baths and muscle-relaxing ointments. In chronic cases botox injection or surgery (lateral internal sphincterotomy) is considered.
Anal Fistula and Perianal Abscess
A perianal abscess is a collection of pus caused by infection of the glands around the anus; it presents with severe pain, swelling and fever and requires urgent drainage. After drainage, a group of patients develop a permanent tunnel (fistula) between the skin and the anal canal. Treatment options include fistulotomy, seton placement, the LIFT technique and laser closure.
Pilonidal Sinus
A chronic condition at the tailbone caused by broken hairs becoming buried under the skin. It is most common between the ages of 15 and 30. Non-surgical phenol treatment and the laser method are used in early disease; in extensive and recurrent disease, leaving the wound open or flap techniques (Limberg, Karydakis) are preferred. Laser hair removal after treatment is the most effective measure against recurrence.
When to See a Doctor
Warning: Not every episode of rectal bleeding is caused by haemorrhoids. Colonic polyps and cancers can bleed in a similar way. If you are over 45, if your bowel habit has changed, or if you have unintended weight loss or anaemia, see a doctor. Dismissing bleeding as "definitely just piles" can mean missing the chance of an early diagnosis.

It depends on the method. Laser and Doppler-guided techniques involve no incision or sutures, so pain is markedly less and recovery takes 3-7 days. Conventional haemorrhoidectomy has a longer and more painful recovery but the lowest recurrence rate.
Is rectal bleeding always haemorrhoids?
No. Colonic polyps and cancers can also bleed. Assessment is essential, particularly over the age of 45 or where there is a change in bowel habit, weight loss or anaemia.
Can pilonidal sinus be treated without surgery?
In early-stage, limited disease phenol and laser methods give good results. In extensive, multi-tract and recurrent disease surgical methods come to the fore.
