Endoscopy is the internal examination of the digestive system with a flexible instrument carrying a camera at its tip. It is used for both diagnosis and treatment and is an integral part of general surgical practice.

Gastroscopy

Entering through the mouth, the oesophagus, stomach and duodenum are assessed. The main indications are:

Colonoscopy

  • Colorectal cancer screening (generally from 45-50 years of age)
  • Rectal bleeding, positive faecal occult blood test
  • Change in bowel habit
  • Unexplained iron deficiency anaemia
  • Follow-up in those with a history of polyps or cancer
  • Assessment of inflammatory bowel disease

Polyps found during the procedure can be removed in the same session. This is the most effective way of preventing colorectal cancer.

Endoscopic Treatments

  • Polypectomy: endoscopic removal of polyps
  • Control of bleeding: stopping ulcer or vessel bleeding with clips, injection or cautery
  • Stent placement: to restore passage where a tumour causes a stricture
  • Endoscopic gastric balloon: a balloon placed in the stomach to restrict volume; temporary weight-loss support for patients who have not reached the surgical threshold
  • Gastric botox: botulinum toxin applied to the stomach muscle to slow gastric emptying
  • ERCP: an advanced endoscopic procedure for the bile and pancreatic ducts

Before and After the Procedure

Before gastroscopy, fasting for 6-8 hours is usually sufficient. Before colonoscopy the bowel must be cleared, which requires special preparations and a set diet. When performed under sedation, patients feel no discomfort.

You should not drive until the effects of sedation have worn off. If a polyp has been removed, avoiding strenuous activity for a few days is advised.

Is endoscopy painful?

When carried out under sedation, patients feel no discomfort and usually do not remember the procedure. Without sedation, gastroscopy can cause a temporary gagging sensation.

What do I need to do before a colonoscopy?

The bowel must be completely cleared. This requires a special diet the day before and bowel-cleansing preparations. If preparation is inadequate the procedure may have to be repeated.

Is a gastric balloon an operation?

No. The gastric balloon is placed endoscopically with no incision. It usually stays in the stomach for 6-12 months and is then removed. It is a temporary option for patients who have not reached the surgical threshold.
