Insurance and Coverage for Surgery in Turkey: How It Works

The General Framework

For those insured in Turkey, most general surgical operations are covered by the Social Security Institution (SGK). Hernia, gallbladder, appendicitis, thyroid, haemorrhoid, pilonidal sinus and oncological procedures are defined in the Health Implementation Communiqué.

No referral is required to attend a private institution contracted with SGK; an appointment can be made directly. Whether an institution holds an SGK contract should be confirmed with that institution before you attend.

If you are not insured in Turkey, SGK does not apply. In that case arrangements are made directly with the hospital, and international patients are usually handled by the hospital's international patients department. Contribution and Additional Fee

Two separate items are often confused.

The contribution is a share defined in the regulations and collected by the institution for consultations and certain procedures.

The additional fee is the amount that private institutions contracted with SGK may request on top of what the institution pays. It works as follows:

  • Its upper limit is set by Article 73 of Law No. 5510; it cannot exceed a defined multiple of the amount the institution pays.
  • Charging it is not compulsory; an institution may choose not to.
  • The patient must be informed before the procedure. You have the right to know the amount in advance.

In some situations an additional fee cannot be charged. Emergencies and headings such as intensive care are regulated separately. Ask the admissions department which category your own case falls into.

Conditions Required in Bariatric Surgery

In terms of coverage, bariatric surgery differs from other general surgical operations and is subject to additional conditions. The framework defined in the Health Implementation Communiqué consists of the following headings:

  • For patients with a body mass index of 40 and above, the necessity of the operation must be documented by a health board report that includes an endocrinologist.
  • For patients with a body mass index between 35 and 40, specified accompanying conditions must be present. The headings listed in the regulations include coronary artery disease, diabetes mellitus, hyperlipidaemia, hypertension, sleep apnoea, and degenerative osteoarthritis and vertebral disc disorders causing restricted movement.
  • For this second group it must also be documented that weight has not been lost despite at least six months of lifestyle change or medical treatment under specialist supervision.
  • The operation must be performed in a centre authorised by the Ministry of Health.

These provisions are updated over time. For the conditions in force at the time you apply, information should be obtained from the institution.

The Health Board Report

Where a board report is required, the process generally consists of a general surgical assessment, an endocrinology assessment, a dietetic consultation, a psychiatric assessment and completion of the necessary tests. The results are considered together and the report is issued.

Private Health Insurance

Private health insurance operates independently of SGK and its rules vary by policy. The headings that most often cause difficulty are:

  • Waiting period. Most policies define a waiting period from inception for certain operations.
  • Pre-existing conditions. Conditions present before the policy started are excluded under most policies, which is why a complete declaration at application matters.
  • Pre-authorisation. Approval must be obtained from the insurer before surgery. This is usually handled by the relevant department of the institution, but it is in your interest to follow it up.
  • Network institutions. The list of institutions your policy contracts with directly affects the proportion covered.
  • Exclusions. These vary by policy; bariatric surgery is separately regulated under some policies.

Confirming your cover in writing from your insurer is safer than relying on verbal information.

Questions to Ask Before You Attend

  • Does the institution hold an SGK contract?
  • Will an additional fee be charged for the planned procedure, and if so how much?
  • Are admission, surgery, anaesthesia and pathology assessed separately?
  • Is a health board report required, and if so which departments must assess me?
  • If I have private insurance, when will pre-authorisation be obtained?
  • Are postoperative check ups and tests included?
